Before filling in this form, please take note of the provisions of Articles 24 and 25 of the Rules of Procedure of the Administrative Tribunal.

Once completed, the form must be printed out and signed by hand. The signed form must be sent to the Registry, both in its original paper form and in scanned digital form, in accordance with the procedures set out in Article 9 of the Rules of the Administrative Tribunal.

Anyone wishing to submit a request for rectification, revision, interpretation and execution in person, or hand in documents, is invited to contact the Registry beforehand to arrange an appointment in the offices of the Registry at the Palais de l'Europe.
